Alle afspraken met en voor een sessie met Laxman Raj Giri zijn gemaakt op basis vanuit vrije wil. Voorafgaande aan de intake zal u gevraagd worden een registratieformulier in te vullen. De sessies kunnen niet gezien worden als medische behandeling of ter vervanging worden beschouwd. De cliënt/participant is zelf volledig verantwoordelijk voor het resultaat gedurende en na de sessie. Minderjarige moeten begeleid worden door een ouder of aangewezen voogd.
Heeft u ons iets belangrijks te vertellen zoals hartritmestoornis, hyperventilatie, psychose, epilepsie (ook als u al lange tijd zonder medicatie bent), hoge sensitiviteit in lichaam en geest of iets anders wat voor u van belang is, schroom dan niet om dit kenbaar te maken want wij houden hier graag rekening mee. Wij hoeven niet te weten wat uw medische dossier is (zie privacy specificatie), maar willen zelf ook niet voor verrassingen komen te staan. U bent niet verplicht deze informatie met ons te delen. Mensen met een bepaalde gevoeligheid voor prikkels kunnen wel sterk op de sessie van Laxman Raj Giri e.o. zijn energie reageren. Wij zijn niet in het bezit van een EHBO-diploma hiervoor. Zoals op onze website geadviseerd wordt kunt u een persoonlijke begeleider meenemen naar de sessie. U kunt ons hier dan ook niet voor verantwoordelijk stellen als u op uw beurt ons niet heeft geïnformeerd. Heeft u vragen over u persoonlijke medische conditie dan bespreekt u dit éérst met uw huisarts. Zo nodig starten wij gezamenlijk een trauma-traject op zodat vanaf verschillende kanten uw welzijn gemeten en beoordeelt kan worden. Deel ook met ons uw beleving ná de sessie. Dit is voor ons van belang zodat wij beter inzicht krijgen in uw proces en u hierbij kunnen begeleiden. Wij zullen op onze beurt onze uiterste best doen u zo goed mogelijk te begeleiden.
Wij adviseren niet binnen twist-zaken, maar ondersteunen de persoon die bij ons een hulpvraag stelt volgens onze richtlijnen van een individuele sessie. Wel behouden wij ons het recht voor om een individuele sessie te beëindigen op basis van twist met een derde persoon. Houdt u hier ook rekening mee bij een begeleidde traject voor minderjarige of bij trauma. Ondanks dat Mantra Shakti niet aangesloten is bij een beroepsvereniging hanteren wij voor onszelf de regel ‘pluis of niet pluis’. In dat geval verwijzen wij u door om zelf contact op te nemen met een gespecialiseerde therapeut of arts. Dit advies noteren wij voor onszelf op uw formulier. Belevingen kunnen per persoon van elkaar verschillen. Laxman Raj Giri zal zich volledig inzetten voor het begeleiden in uw vragen, maar behoud zich het recht voor een sessie/training uit vakkundig oogpunt te doen beëindigen.

Privacy: In verband met de nieuwe AVG privacy wetgeving in Europa (25-5-2018).
UW PERSOONLIJKE INFORMATIE: De persoonlijke informatie die u invult op ons cliënt registratie formulier zijn uitsluitend bedoelt voor het goed kunnen begeleiden in het persoonlijke proces waarvoor u gekozen hebt om dit met Mantra Shakti aan te gaan. Wij vragen niet naar uw medische achtergrond, maar zijn wel graag door uzelf geïnformeerd naar lichamelijke en mentale zwakheden zodat wij daar rekening mee kunnen houden. Contactgegevens zoals uw telefoonnummer zullen wij alleen gebruiken ivm de planning omtrent uw afspraak, of als u in een situatie zelf aangeeft vanaf een andere locatie, dan die van Mantra Shakti, geholpen te willen worden. Andere informatie omtrent uw (privé) voorkeuren zijn voor ons niet relevant voor de sessie. Wat u met verworvenheden uit een sessie met Laxman Raj Giri doet is uw eigen aangelegenheid en verantwoordelijkheid.
VERWERKING VAN PERSOONSGEGEVENS: Wilt u uw gegevens bij ons inzien dan kan dit op afspraak. Alle gegevens die op papier zijn ingevuld worden bij ons niet gedigitaliseerd. Informatie uit uw sessie welke dient voor uw proces begeleiding noteert Laxman Raj Giri in het Nepalees op de achterzijde van uw formulier en kunnen door hem mondeling worden toegelicht. Wij bewaren dit 5 jaar. Mocht u na 5 jaar wederom contact met ons opnemen dan zal het cliënt registratie formulier wederom moeten worden ingevuld. Alle digitale tekst wordt binnen een jaar verwijderd, mits het betrekking heeft op uw dossier en zullen u hiervan op de hoogte stellen. In dat geval printen wij dit uit en wordt dit 5 jaar bij het dossier bewaard. Is er documentatie over een onderlinge onenigheid/danwel twist dan zullen wij deze documentatie toevoegen aan uw dossier en dit in zijn geheel 25 jaar bewaren tot aan verjaring die staat voor een juridische procedure. De (normale) digitale mail- en multimedia wisseling zal binnen 1 jaar worden verwijderd. Foto’s en videomateriaal was/is/wordt gemaakt met uw goedkeuring zal digitaal worden bewaard zonder verdere vervaldatum.
Bent u van gedachte veranderd en wilt u informatie verwijderd hebben laat ons dit dan schriftelijk weten zodat wij dit ook aan u terug kunnen bevestigen. Zonder schriftelijk verzoek met daarin een door u helder gestelde vraag / en uw verwachting kunnen wij u niet van dienst zijn. Wederzijdse verwachtingen worden in het begin van uw sessie bij het intakegesprek mondeling besproken naar aanleiding van het vrije veld op ons cliënt registratie formulier.
PLUIS OF NIET PLUIS: Ondanks dat Mantra Shakti niet aangesloten is bij een beroepsvereniging hanteren wij voor onszelf de regel ‘pluis of niet pluis’. Niet pluis betekend dat er door ervaring bij ons een vermoeden ontstaat dat er iets mis is met uw gesteldheid (gezondheid of emotioneel/sociale belevingswereld). In dat geval verwijzen wij u door om zelf contact op te nemen met een gespecialiseerde therapeut of arts. Dit advies noteren wij voor onszelf op uw formulier. In het uiterste geval beëindigen wij zelf uw traject met ons, met of zonder verdere toelichting.

FACTUUR: Wilt u een van de volgende producten en/of daar een factuur van ontvangen, dan moet u volgens de nieuwe wetgeving zelf uw contactgegevens aan ons doorsturen. Dit kan via ons contactformulier, of per e-mail.

If you have any problems understanding the information on our website, as it is meanly written in Dutch, please send us an email so we can confirm this to you in writing. No asked questions do not count.
All appointments with and for a session with Laxman Raj Giri are made e based on free will. You will be asked to fill in a client registration form before starting your first session. All clients/participants and related persons should be aware that this session does not replace nor can be seen as any kind of medical treatment. The client/participant is fully responsible for the result during and after the session with Laxman Raj Giri. Under aged must be accompanied by a parent or an appointed guardian.
Do you have something important to tell us, such as heart rhythm disorder, hyperventilation, psychosis, epilepsy (also when you are without medicine for some time), high sensitive in body and mind or anything else which may feel important to you, please do not hesitate to inform us because we like to take this into account as much as possible. You are not required to share this information with us. People with a particular sensitivity are known to react strong on the energy-session of Laxman Raj Giri. We are not in the possession of a first aid diploma if anything does occur. As advised on our website you can get a personal attendant during the sessions. Someone who looks after you in case you do not feel well. If you choose not to inform us of your personal (or in case of your child) medical, psychological of high sensitive situation we can not held responsible. Do you have questions about personal medical condition then you must discuss this with your doctor first! If necessary, we prefer to start a trauma-session together with your physician/doctor/therapist of coach. Share with us your experience after the session. This is important to us so that we better understand your process and can guide you. For our part we will do our best to guide you.
We will not interfere in any kind of disputed between you and any other person. We will guide according with our guidelines for an individual session. However, based on disputed/quarrel with a third person we may cancel our participation and terminated our assistance for private healing. Please also note here minors and trauma-sessions. Despite that Mantra Shakti is not affiliated with a professional alternative healing association, we use our own knowledge to intepretate your wellbeing; in Dutch called ‘pluis of nit pluis’. Meaning; In that case we suspect health issues, physically or mentally, we will request you to consult a specialized/therapist or doctor. Our advices to you will be noted on your client registration form. Experiences can differ per person. Laxman Raj Giri will fully committed himself of guiding you at level best, but reserves the right to terminate a session/training from professional point of view.

PERSONAL INFORMATION: The personal information you filled in on our client registration at Mantra Shakti is exclusively intended for therapist guidance in your personal process for which you have chosen Mantra Shakti. Contact details such as your phone number, will only be use regarding the planning about your appointment. We do not ask to your medical background, but we do like to be informed by yourself about your physical and mentally weaknesses so that we can keep this in mind. Contact details such as your phone number, will only be use in case the planning/rescheduling of your appointment, or if you are in a situation up from another location, than that of Mantra Shakti, and request us to assist you (distance healing). Other information about your (private) preferences are not relevant to the session. The effect of a session with Laxman Raj Giri does stand with your own actions regarding to establish a healthier life and is your own responsibility.
PROCESSING OF PERSONAL DATA: If you want to see your information with us then this can be done only may by scheduling an appointment. All the data are filled in on paper are not digitalized. Information about the happing during/reagrding your session will be note by Laxman Raj Giri in Nepali on the backside of your form and can be explained by him orally. We will retain this 5 years. Should you contact us again after 5 years then the client registration form should be filled in again. Is there documentation on a mutual disagreement/or twist we will add this documentation to your file and keep until this completely for 25 years as like a legal process. The (normal) digital mail-and multimedia exchange will be removed within 1 year. Photos and video material which whas/is/ will be created with your approval will digitally be kept without any end date.
Have you changed your mind and do you want to have information been removed, please let us know in writing so we can confirm to you. Without written request containing a clear question asked by you/and your expectation we do not act. Mutual expectations will be talked over at the beginning of your intake session and reviewed at the end. Please fill in your expectations in the free field on our client registration form.
‘PLUIS OF NIET PLUIS’ meaning WHEN DO WE THINK SOMETHINGS WRONG: Despite Mantra Shakti is not affiliated with a professional alternative healing association we do use ourselves the Dutch understanding called ‘luis of niet pluis’. Means; if suspicion arises we think there is something wrong with your condition (health or emotional/social), we request you to consult a specialized therapist or doctor. This advice will be note on your form. In the extreme case we do keep the right to terminate your session with us immediately, with or without further explanation.

INVOICE: Do you like to receive an invoice for one of the following products and/or receive an invoice, according to the new legislation you have to inform us about your personal en company details yourself. This can be done via our contact form, or by email.
- Do you want to receive an invoice: Personal-and/or company name, full address (invoice request a few days before your appointment)
- Would you like us to read out your personal Vedic Horoscope: Your name, exact birth time (the minute), birth place (including hospital in another city)
- Would you like us to perform a House Enrichment: The names from the family who live at this address (or still as a child at College settled at home sleeping) for first and last name, full address
- Do you want personal protection with a locket: We ask all your given birth names (fully written out) and date of birth
